FINANCIAL & CLINIC POLICIES AGREEMENT

This Financial and Clinic Policies Agreement (“Agreement”) outlines the financial terms, operational rules, and responsibilities of all patients receiving services at New Regeneration Health Medical Center. By signing below, you acknowledge and agree to comply with the following policies:

Payment Policy

• Payment Due at Time of Service

Full payment is required on the same day that any service or procedure is performed. We accept credit/debit cards, cash, and certain third-party financing services (where applicable).

• Invalid or Fraudulent Payment

Any form of payment that is denied, reversed, or determined to be fraudulent will result in immediate suspension of services. The clinic reserves the right to contact law enforcement authorities if necessary.

Cancellation, Late Arrival, and No-Show Policy

We value your time and reserve your appointment exclusively for you. To respect all patients and providers:

• Cancellations must be made at least 24 hours in advance.

Cancellations made with less than 24 hours' notice may be subject to a cancellation fee of $50 or more, depending on the nature of the appointment.

• No-Show Policy:

Patients who fail to appear for a scheduled appointment without prior notice will be considered a “no-show” and may be charged a fee of up to 100% of the scheduled service cost.

• Late Arrivals:

Arriving more than 15 minutes late may result in rescheduling or modification of your treatment and may incur a late fee.

• Patients who accumulate more than two no-shows or repeated late cancellations may be restricted from booking future appointments without prepayment.

Membership & Package Policies

• Prepaid Packages and Memberships:

All membership plans and treatment packages are non-transferable and non-refundable. Package sessions must be completed within the stated expiration period from the date of purchase (typically 6 or 12 months).

• Terms of Use:

By purchasing a membership or package, you agree to the individual Membership Agreement, which will include usage rules, expiration timelines, and policies for missed or rescheduled sessions.

• Violation or Abuse of Membership Terms:

May result in cancellation of membership benefits without refund.

Refund Policy

• All Sales Are Final.

All services rendered, products sold in office or from the New Regeneration Website, or treatments prepaid are final and non-refundable. This includes but is not limited to:

  - Skincare or health products (cosmetics, vitamins, nutraceuticals, etc.)

  - Prepaid procedures or treatment sessions (regardless of usage)

  - Gift cards or promotional purchases

We do not offer refunds for dissatisfaction with results, as outcomes may vary based on individual response.

Consent and Acknowledgment

By continuing to receive care at New Regeneration Health Medical Center, patients acknowledge that they have reviewed and understand the clinic’s policies regarding scheduling, cancellations, payments, and refunds.

Patients are responsible for all financial obligations related to their treatment and authorize New Regeneration Health Medical Center to process payments for outstanding balances using the payment information provided.

Our goal is to maintain transparency, mutual respect, and a clear understanding between our clinic and our patients to ensure a smooth and positive care experience.
